It provides access to discounted dental services at Clay Center Family Dental Care and does not meet minimum essential coverage requirements. It cannot be combined with other insurance, discount plans, or third-party payment arrangements. Membership is specific to one person and is non-transferable. Unused covered services do not roll over and have no cash value. The plan renews automatically each year unless canceled in accordance with the membership agreement. Members are responsible for payment of any services not covered by the plan.
†Monthly payments require that a credit card be put on file. The first 3 payments will be charged at the time of purchase. The remaining balance will be charged evenly on a monthly basis starting on the 4th month of the membership plan.
